Ikuaikai Ladies’ Clinic
New Year’s Greetings
2026/01/05
Happy New Year.
Ikuaikai Ladies’ Clinic celebrates its 69th year in 2026 since it was founded by my grandfather, Dr. Shoji Sugishita. We will continue to provide gynecological care rooted in the local community and appreciate your continued support.
Dr. Yodo Sugishita, who has been working at our clinic on a part-time basis and is also my sibling, has recently retired from St. Marianna University School of Medicine(His specialties are adolescent endocrinology, reproductive medicine, and menopausal medicine — in short, all areas related to hormones.) From now on, he will be able to support our clinic even more than before.
We sincerely look forward to your continued patronage this year as well.

a) Hormone Replacement Therapy: For Those Concerned About Menopause
During menopause, individuals may experience a wide variety of symptoms. By choosing treatment methods tailored to each specific symptom, it is often possible to eliminate or alleviate discomfort and help you maintain a comfortable daily life. If you are experiencing symptoms, we recommend that you consult a doctor as early as possible.
Causes of Menopausal Symptoms
“Menopause” refers to a period of roughly 10 years around the time of the final menstrual period, which most people reach in their late 40s to early 50s. During this time, the secretion of female hormones declines rapidly, leading to various physical and emotional changes. Some people feel depressed due to symptoms that interfere with daily life or symptoms they have never experienced before. However, menopause is a natural life stage that all women go through. Rather than feeling excessively discouraged, let us work together to find ways to relieve your symptoms.
Common Menopausal Symptoms
• Hot flashes: a sudden feeling of warmth in the face, flushing, or a sensation of heat despite the ambient temperature, often accompanied by sudden sweating.
• Palpitations.
• Cold hands and feet, even when it is not cold.
• Severe stiff shoulders.
• Irritability and feelings of anxiety.
• Tendency toward depressed mood.
• Difficulty falling asleep or poor-quality sleep (insomnia).
We use the Simplified Menopausal Index (SMI score) to easily assess the severity of your menopausal symptoms. This score is not an absolute diagnosis, but it can serve as a guide in deciding whether to seek medical care.
Scoring Criteria
Severe: Symptoms are distressing enough to interfere with daily life; you want relief immediately.
Moderate: Symptoms are tolerable but you would like some relief.
Mild: You notice symptoms, but they are manageable.
None: You have little to no symptoms.
Scoring Criteria
| Symptom | Severe | Moderate | Mild | None |
|---|---|---|---|---|
| 1.Hot flashes/flushing of the face | 10 | 6 | 3 | 0 |
| 2.Excessive sweating | 10 | 6 | 3 | 0 |
| 3.Tendency to feel cold in the lower back, hands, or feet | 14 | 9 | 5 | 0 |
| 4.Shortness of breath or palpitations | 12 | 8 | 4 | 0 |
| 5.Difficulty falling asleep or shallow sleep | 14 | 9 | 5 | 0 |
| 6.Irritability or becoming easily angered | 12 | 8 | 4 | 0 |
| 7.Tendency to brood or feel depressed | 7 | 5 | 3 | 0 |
| 8.Frequent headaches, dizziness, or nausea | 7 | 5 | 3 | 0 |
| 9.Fatigue or tiredness | 7 | 4 | 2 | 0 |
| 10.Shoulder stiffness, low back pain, or pain in the limbs | 7 | 5 | 3 | 0 |
Self‑Assessment Guidelines
0–25 points: You are coping well with menopause; no particular problem at present. Please have a health checkup once a year.
26–50 points: Pay attention to diet, sleep, and exercise, and try to maintain a balanced lifestyle. If you begin to experience any particularly troublesome symptoms, please reassess your score.
51–65 points: A consultation with a physician is recommended. Please visit us to discuss starting medication or other treatments.
66–80 points: Planned, longer‑term treatment (for at least six months) is likely to be necessary. Please visit us for a consultation.
81–100 points: You may need detailed examinations in multiple specialties. Because tests may be required not only in gynecology but also in other departments, please visit us to discuss this. Long‑term treatment will probably be required.
